  • IN CASE OF DEATH OF A GOVERNMENT SERVANT FOLLOWING SHOULD BE SUBMITTED TO COMMUNICATION ACCOUNTS OFFICE THROUGH AO (TA) OF THE SSA CONCERNED. ALL DOCUMENTS ARE TO BE COUNTERSIGNED BY HEAD OF THE OFFICE OR ANY OTHER OFFICER SO ENTRUSTED BY HEAD OF THE OFFICE.

  • Form 14 -- Application for grant of Family Pension.

  • Form 12 – Application for grant of death gratuity.

  • Form 18 – For assessing and authorishing payment of family pension and death gratuity when a govt. servant dies while in service.

  • Service Book indicating date of death.

  • Two specimen signatures or left hand thumb and finger impression of the claimant or guardian ( in case of minor) duly attested.

  • Two copies of passport size photographs of the claimant or guardian duly attested.

  • Two copies of descriptive roll of the claimant or guardian duly attested indicating height and personal marks.

  • Postal Address of the claimant.
